add MAKEWPARAM and FIRSTBYTE~FOURTHBYTE macros, and remove HIBYTE_WORD16, LOBYTE_WORD16 macros. Now, MAKEWORD(lobyte, hibyte) returns same value on both 32-bit and 64-bit platforms
